How ketamine treats depression

Glutamate, an excitatory neurotransmitter in the brain, regulates large regions of our central nervous system. Glutamate is thought to play a key role in the development of depression. Stress and other factors can alter glutamate signaling in the brain and alter neurons to make them less resilient and less able to communicate with other neurons. Ketamine alters glutamate transmission which is thought to contribute to its ability to alleviate depression.

What is Depression?

Depression is a mental health disorder caused by a variety of factors like genetic predisposition, chemical imbalances in the brain, environmental stressors and life events such as trauma or loss. Symptoms vary based on the type of depression and the individual.

Common Depression Diagnoses

The Diagnostic and Statistical Manual of Mental Disorders (DSM-5) recognizes several types of depression. Common types of depression include:

  • Major Depressive Disorder (MDD): MDD is characterized by a persistent feeling of sadness or loss of interest in activities that a person used to enjoy. Other symptoms of MDD may include changes in appetite, sleep disturbances, fatigue, difficulty concentrating and thoughts of self-harm or suicide.
  • Persistent Depressive Disorder (PDD): PDD, also known as dysthymia, involves chronic and persistent symptoms of depression lasting for at least 2 years in adults or 1 year in children.
  • Postpartum Depression (PPD): PPD is a type of depression occurring after giving birth and is characterized by feelings of sadness, anxiety, or hopelessness, changes in appetite or sleep patterns and difficulty bonding with the baby.
  • Seasonal Affective Disorder (SAD): SAD is a type of depression that typically occurs during the winter months when there is less natural sunlight. Symptoms can include fatigue, weight gain and difficulty sleeping.
  • Bipolar Disorder: Bipolar disorder is a mood disorder that involves episodes of both depression and mania or hypomania, which can include symptoms like increased energy, racing thoughts and impulsive behavior.

 

Your medical team at Reset Restore MD will use the screening tool PHQ-9 to assess for the presence and severity of depression. The Patient Health Questionnaire (PHQ) scores each of the 9 DSM-V criteria for depression. This 9-item scale will be used before and upon completion of IV ketamine treatment. 

Depression and Suicide

Those with depression may experience suicidality. The American Psychological Association defines suicidality as “the risk of suicide, usually indicated by suicidal ideation or intent, especially as evident in the presence of a well-elaborated suicidal plan.” It can also be defined to include suicidal thoughts, plans, gestures, or attempts. In 2021, suicide was the 2nd leading cause of death for people ages 20-34.

A 2023 systematic review, concluded ketamine treatment resulted in a significant reduction of suicidal ideation. Additionally, the systematic review demonstrated the superiority to IV ketamine versus intranasal. Full suicidal remission was reached in 30-60% of cases.
